Membership
$150/month, age 21 to 64.
Gives access to telehealth and home visits 24 hours a day/7 days a week. All primary care services to include physicals, age recommended preventative health screenings, annual labs, follow-up, care personalized to you and your health needs. Telehealth visits are included in this plan at no additional charge. Home visits, as needed, will be $100/visit. Advanced lab testing is included in this level of care. Note: All labs ordered are billed separately of the monthly membership fee.
All members have access to pharmaceutical quality nutritionals @ a 10% discount Treatment is based on an integrative approach, that includes not only traditional medication therapy, but also nutritional supplementation, dietary education/suggestions, stress management therapies, etc
75 minute in person intake – $500
Your initial intake visit is a comprehensive, in person intake. This includes delving into your full medical history from birth to present, and a physical examination. From this, we will develop an initial treatment plan that may include basic and advanced lab testing, dietary and/or lifestyle changes, and possible nutritional supplements if needed. Prescription medications may be needed also. This visit, and the care plan that comes from it, takes considerable time to complete, which is more than just the visit time. This will be a comprehensive plan to set the foundation of everything else we will partner on. It’s the first, crucial step on your journey to optimal health.
After posted office hours, we ask you to not contact the provider if it is something that can wait until the next day. As always, if you feel your problem is a medical emergency, please call 911 or go to your local emergency department.
NOTE: Bespoke Health does not provide narcotic pain medications, and will not refill ANY narcotic pain medications you may currently be taking. We will be happy to refer you to a pain management specialist, if that is what you desire. THERE ARE NO EXCEPTIONS TO THIS. Any patient demanding narcotic pain medications who is unwilling to see a pain management specialist will be discharged from the practice.
