Health insurance is for disease management and catastrophic illness and it is NOT for health optimization. If you would like insurance-catering healthcare please go elsewhere.

The labs we do, the nutrients we advise, the lifestyle interventions we suggest MAY NOT COVERED BY INSURANCE. Medicines we prescribe may still be covered. However we do have affordable methods to optimize health available.

Rockerfeller-based petrochemical medicine keeps you sick. We were taught this way in school and understand that it is NOT moral or ethical to practice this way. We want you so healthy and powerful with life that you never need sick-care medicine or develop common disease of age. Our patients become super healthy and full of life. For this reason we are a subscription based practice. Sick-care medicine prefers they bill insurance higher for multiple ICD-10 codes at your visit and see you more often and inject you with more things at every visit so they can add CPT codes for procedures and diagnose more diseases to bill your insurance higher. Please do not ask us to invent 10 undiagnosed disease codes to your chart so you can try to bill your insurance for labs.

MEMBERSHIP OPTION:$150/month, age 21 to 64.

Membership includes quarterly visits & as needed weekdays, 30 – 45 minute visits.

Gives access to telehealth and home visits 24 hours a day/7 days a week. All primary care services to include physicals, age recommended preventative health screenings, annual labs, follow-up, care personalized to you and your health needs. Telehealth visits are included in this plan at no additional charge. Home visits, as needed, will be $100/visit. Advanced lab testing is included in this level of care. Note: All labs ordered are billed separately of the monthly membership fee. The cost of all lab testing is deeply discounted for members.

All members have access to pharmaceutical quality nutritional supplements @ a 10% discount Treatment is based on an integrative approach, that includes not only traditional medication therapy, but also nutritional supplementation, dietary education/suggestions, stress management therapies, etc.

Note, your first visit will be schedule for at least one hour. This visit will be a comprehensive evaluation of you, and in most cases, be done in person.

PAY AS YOU GO OPTIONS:

NEW PATIENT: $150 Average appointment length is 45-60 minutes spent together. NEW PATIENT PAPERWORK MUST BE RECEIVED PRIOR TO SCHEDULING.

Established patient up to 30 minutes: $80

Established patient up to 60 minutes: $150

Brief visit established patient up to 10 minutes: $50

Sick visit new or established patient: $75

Note: 

Non members pay regular prices for all labs and any nutritional supplements, and do not have access to texting for follow ups. Any follow up will be charged at the above prices. 

Lab testing is billed separately from the visit fee. 

Home visits for non members are the above prices with an additional $100 convenience expense. 

VISIT FEES FOR NON MEMBERS MUST BE PAID PRIOR TO THE VISIT. NO EXCEPTIONS. 

NOTE: Bespoke Health does not provide narcotic pain medications, and will not refill ANY narcotic pain medications you may currently be taking. We will be happy to refer you to a pain management specialist, if that is what you desire. THERE ARE NO EXCEPTIONS TO THIS. Any patient demanding narcotic pain medications who is unwilling to see a pain management specialist will be discharged from the practice.
